How to download Wix form submissions as PDFs, in bulk and with your logo

A clinic gets 40 intake forms in a week and the manager wants them in a folder, one PDF each, with the clinic's logo on top. In Wix Forms you open a submission, press download, and repeat that 39 more times.
That's how Wix built it. The help article on the submissions table says: "You can only download one submission at a time." And: "You can't customize the PDF settings." It adds: "Only fields with data are included in the PDF." The only bulk export it describes is CSV, and there "all dates and times are exported in UTC".
Wix has a feature request open for the layout side. The page says: "Currently, it is not possible to customize PDF layouts for form submissions." It's marked "We are collecting votes for this issue", and it names no workaround.
What people do today
Download the PDFs one by one and rename each file by hand. Or export the CSV and rebuild a nice-looking document in Sheets or Word with a mail merge. Both work for 5 submissions. At 40 you've lost an hour.
Uploaded files are a second trap. Another request page says: "Currently, files that visitors upload using Wix Forms are sent directly to your email address". They aren't saved on the contact card, and that page is also collecting votes. So the signed consent and the photo end up in a mailbox, away from the form answers.
Form PDF makes the PDFs for you
I built Form PDF for this. It's on the Wix App Market.
You pick a form and see its submissions in a table you can sort, search, and filter by date range or by an answer. Press PDF on a row and you get that submission with your business name and logo, minus any fields you told it to leave out. Signatures and PNG or JPG uploads are drawn into the PDF. Premium adds your colour, a footer and the field order.
The bulk part: tick the rows, or a date range, and download them as one merged PDF or as a ZIP of separate files. Premium can also file a PDF on the contact card, so the submission sits next to the contact's other details. In my test on a dev site, a new submission showed up as a PDF in the contact's Files in about 10 seconds.
The free plan makes single PDFs from one form (the first form you make a PDF from), with your logo and your choice of fields, plus the sortable table. Premium is $6 a month or $60 a year ($5 a month billed yearly): bulk download, every form on the site, colour, footer, field order, and filing on contact cards.
Two limits. Right-to-left and Chinese, Japanese and Korean scripts aren't supported yet. And I haven't tested a submission typed by a visitor into a form on a live page. My test submissions went through the same upload path the form widget uses, but a visitor typing it in is a different thing. The app needs Wix Forms, the newer Forms app, with at least one form that has submissions.
